Dr. Cooper's office is committed to making the administrative aspects of your care as straightforward as possible. Below you will find information about insurance, billing, and financial expectations for your visits and procedures.
Insurance
Dr. Cooper participates with most major commercial insurance plans. We recommend contacting your insurance carrier directly before your appointment to verify that Dr. Cooper is in-network with your specific plan and to understand your deductible, co-pay, and co-insurance responsibilities. Insurance plan networks can change, so verifying coverage prior to each visit is always advisable.

Prior Authorization
Many insurance plans require prior authorization for specialist visits, advanced imaging (MRI, CT), and surgical procedures. Our office will handle prior authorization requests on your behalf and communicate with your insurer to facilitate approval. Please be aware that authorization does not guarantee payment — your insurance company makes final coverage determinations at the time of claim processing.
What to Bring to Your Appointment
Please bring your current insurance card(s), a valid photo ID, and any referral documentation required by your plan to every appointment. If your insurance information has changed since your last visit, please notify our front desk staff as soon as possible.
Co-Pays and Patient Balances
Co-pays are due at the time of service. Patient balances resulting from deductibles, co-insurance, or non-covered services will be billed to you after your insurance claim is processed. We accept most major credit cards, checks, and cash. Payment plans may be available — please contact our billing department to discuss your options.
Out-of-Network and Self-Pay Patients
If you are out-of-network or do not have insurance, please contact our office prior to your appointment to discuss self-pay rates and financial arrangements. We strive to make high-quality orthopedic care accessible and will work with you to find a solution.
